Setting up child health and mortality prevention surveillance in Ethiopia

Background: Mortality rates for children under five years of age, and stillbirth risks, remain high in parts sub-Saharan Africa South Asia. The Child Health Mortality Prevention Surveillance (CHAMPS) network aims to ascertain causes child death mortality settings (>50 deaths/1000 live-births). We aimed develop a “greenfield” site CHAMPS, based Harar Kersa, Eastern Ethiopia. This very setting (>100 live-births Kersa) had limited previous surveillance capacity, weak infrastructure political instability. Here we describe development, from conception 2015 the end first year recruitment. Methods: formed collaboration between Haramaya University London School Hygiene & Tropical Medicine engaged community, national international partners support new CHAMPS programme. developed laboratory recruited trained staff. established project specific procedures implement protocols including; notifications, clinical demographic data collection, post-mortem minimally invasive tissue sampling, microbiology pathology testing, verbal autopsy. convened an expert local panel determine cause-of-death. In partnership with Ethiopian Public Institute strategies improve maternal health. Results: Despite considerable challenge, financial support, personal commitment, effective partnership, successfully initiated CHAMPS. One into recruitment (February 2020), received 1173 unique investigated 59/99 MITS-eligible cases within site, assigned underlying immediate cause 53 children. Conclusions: most valuable global health policy are high-mortality settings, but initiating has required resource. To further leverage this investment, need strong, sustained, research leadership, broaden scientific remit. this, have set up collaboration, “Hararghe Research Partnership”.
